疑难杂症与免疫系统紊乱综合征理论

我是山西中医杨俊耀,深耕中医临床已有数十年。在长期接诊大量久治不愈、病因模糊的疑难杂症患者时,我跳出传统中医 “辨证分型对号入座” 的固化诊疗思维,结合现代医学对人体免疫功能的相关研究规律,独立构建起 “疑难杂症与免疫系统紊乱综合征” 完整理论体系。这套理论摒弃了以往将各类复杂疑难病症割裂看待的诊疗模式,立足人体免疫整体失衡这一核心,重新梳理各类跨系统疑难症状统一的发病机理,搭建起涵盖病机拆解、辨证施治、长期养护的完整诊疗思路。在众多常规疗法收效甚微的疑难病例中,这套理论都收获了稳定持久的临床疗效,也为中医攻克疑难杂症开辟了全新的核心思路。

一、疑难杂症的共性本质:免疫系统紊乱的整体失衡
数十年临床接诊过程中,我观察到大量现代医学归为 “病因不明” 的疑难杂症存在统一共性:患者症状分散于多个身体系统,既不符合单一脏器典型病变特征,也无法依靠单一疾病诊断完整解释;患者辗转多个科室治疗,症状依旧反复发作,难以根治。这类患者常常同时伴随长期反复皮肤过敏、不明关节酸痛、持续性低热乏力、严重睡眠失调、消化功能紊乱等跨系统表现,各项检查均未提示明确器质性病变,最后只能笼统归为亚健康、植物神经功能紊乱,缺乏针对性有效治疗手段。
据此我明确提出:这类疑难杂症的根本问题,不在于局部脏器损伤,而是人体免疫系统整体功能紊乱。这种紊乱不等同于大众普遍认知的 “免疫力低下”,而是免疫机制全方位失衡:一部分免疫通路过度亢奋,错误攻击自身正常组织,诱发各类无菌炎症、过敏反应;另一部分免疫通路持续受抑,无法及时清除体内代谢垃圾与残留病邪,微小病灶长期滞留、久久不愈。这种全身性免疫失衡会顺着气血经络蔓延至全身各处脏腑系统,最终表现为分布在周身不同部位的各类疑难症状,这也是所有跨系统疑难杂症共有的发病根源。
站在中医角度解读,免疫系统整体紊乱,对应的就是人体正气固有的调和平衡被彻底破坏。传统所说的正气,并非单纯指气血充盈,更看重正气内部动态协调:卫气主体表防御,营气主体内濡养,二者相互制衡、协同运转,人体免疫机能才能维持正常。长期劳累、情志刺激、不当用药、环境毒素持续耗损人体,营卫原本平衡的状态便会彻底崩塌。卫气过亢,会出现过敏、红肿、低热等过度免疫反应;卫气偏弱,便反复感冒、周身疲惫;营气运行失常,则气血瘀堵,引发关节疼痛、组织增生等问题。这种营卫整体失衡,无法单用气虚、血虚、阴虚、阳虚任一单一证型概括,这也是常规辨证分型难以根治此类疑难病症的关键原因。
我在此着重强调,以往不少医者治疗这类疑难杂症,只会针对单一症状局部处置,过敏便单纯抗过敏、关节痛只一味止痛,或是堆砌补益气血方药盲目提升免疫,完全没有触及免疫整体失衡的核心病机。最终只能短暂缓解不适,无法根除病根,甚至过度干预会进一步打乱免疫平衡,致使病情愈发顽固复杂。
二、免疫系统紊乱综合征的三层病机演化规律
通过对数千例疑难杂症患者完整病程的跟踪记录,我将免疫系统紊乱综合征的病机演变划分为由浅入深三个阶段,每个阶段都有专属典型症状与核心病机,完全契合疑难杂症逐步加重的自然发病过程,让纷繁杂乱的复杂症状拥有清晰可循的发展逻辑。
第一层:营卫失和的初乱阶段
这是免疫紊乱最早出现的早期阶段。绝大多数患者都存在明确诱发因素:重度感冒后过量使用抗生素、激素,连续数月熬夜超负荷劳作,或是遭受重大情志打击,直接打破体表营卫之气的协调运行。
本阶段典型表现为反复莫名荨麻疹、皮肤瘙痒,遇风极易感冒,长期低热、畏风怕冷,全身游走性疼痛,各类仪器检查均无明显异常指标。此时免疫紊乱仅停留在体表,尚未侵入内在脏腑,是干预治疗的最佳时期。但绝大多数患者都会被误诊为普通过敏、伤风感冒,长期使用对症压制类药物,将本可随体表向外透散的病邪禁锢体内,推动病症向更深层次发展。
第二层:三焦气血郁滞的中乱阶段
若初乱阶段病邪未能及时透散,便会顺着三焦经络向内侵袭,造成全身气血运行通道大范围壅滞,免疫紊乱从体表扩散至全身各大系统。
此阶段患者症状繁杂,多系统异常同步显现:长期胸闷腹胀、食后难消,大便溏稀或便秘交替;伴随严重失眠焦虑、莫名心慌心悸;全身多处关节出现无菌性疼痛,皮肤反复发作顽固湿疹、皮下结节;部分人出现甲状腺功能异常、自身抗体轻度升高等表现。此时人体免疫彻底失衡,过度活化的免疫细胞持续攻击自身黏膜、关节组织,本该发挥防御作用的免疫通路却处于抑制状态。常规疏肝、健脾、补肾方药,难以疏通全身广泛郁堵的气血通道,病情常年反复发作,迁延数年甚至十余年无法断根。
第三层:阴阳气血逆乱的深乱阶段
中乱阶段病症长期失治,最终造成人体阴阳气血彻底逆乱,免疫机能完全丧失自我调控能力。部分患者会发展为明确自身免疫性疾病,如顽固性类风湿、系统性红斑狼疮、干燥综合征等;还有患者常年重度乏力,日常起身活动都难以完成,周身多发不明结节、组织增生,甚至逐步进展为重症病变。
本阶段病机错综复杂,患者同时兼具重度虚损与深层瘀滞,既畏寒怕冷,又频繁口腔溃疡、燥热上火,单用温阳药立刻上火,单用清热药便腹泻便溏,常规辨证极易陷入两难,也是临床最难处理的一类疑难杂症。
三、调治免疫系统紊乱综合征的核心特色治法
结合免疫系统紊乱综合征三层递进的病机特点,我总结出一套区别于传统常规思路的专属调治体系。这套方案核心思路既不是单纯 “补益免疫”,也不是一味 “压制免疫”,而是以调和营卫、疏通三焦、平复阴阳为根本,循序渐进恢复免疫自身动态平衡,从根源化解各类疑难杂症的核心矛盾。
针对第一层营卫失和的初乱阶段,我确立核心治法为宣通营卫,透邪外出,全程禁用纯补益药与苦寒清热药。临床常轻量配伍桂枝、麻黄,搭配防风、荆芥,疏通体表郁滞的营卫之气,让早期侵入体内的病邪顺着肌表自然向外透散,快速恢复营卫调和。多数处于免疫紊乱早期的患者,服药数周后,反复过敏、低热、畏风等症状完全消退,免疫机能快速回归平衡,直接阻断病情向内进展。
针对第二层三焦气血郁滞的中乱阶段,核心治法定为分消三焦,调气和血,不再局限单一脏腑论治,同步疏通上中下三焦全身壅滞的气血通路。我常用桔梗、枳壳开宣上焦气机,半夏、厚朴化解中焦郁阻,茯苓、泽泻疏导下焦湿浊,再佐少量丹参、川芎温和通利周身血脉,逐步打通全身淤堵的气血通道。本阶段调治不求速效,重在循序渐进疏通通路,让紊乱的免疫细胞回归正常气血运行轨道,停止无序的自身攻击。许多迁延十余年的多系统疑难症状,会在持续调治中逐一缓解,免疫状态逐步趋于稳定。
针对第三层阴阳气血逆乱的深乱阶段,核心治法为寒热并调,平复阴阳,跳出 “非温即凉” 的二元对立治疗思维。我会根据每位患者实际病机,精准配比温阳附子与清热黄连,同步调和体内错乱寒热,辅以多味调和阴阳之品,逐步重建人体阴阳平衡秩序。本阶段调治周期偏长,但只要理顺逆乱阴阳,彻底失控的免疫功能就能慢慢恢复自主调节能力。不少重度自身免疫病患者,经长期调治后,可逐步脱离激素、免疫抑制剂依赖,实现病情长期稳定不复发。
我提出的这套疑难杂症与免疫系统紊乱综合征理论,打破了过往各类跨系统疑难病分割诊治的局限,立足人体免疫整体平衡建立统一病机认知与完整调治体系,让众多以往无对症治法的疑难病症,拥有清晰、可落地的治愈思路,也为中医疑难病诊疗开辟了全新学术方向。
I am Yang Junyao, a traditional Chinese medicine (TCM) practitioner from Shanxi with decades of clinical experience. Over many years of treating large numbers of patients with stubborn, difficult-to-cure, and etiologically vague疑难杂症 (difficult and complicated diseases), I have stepped beyond the rigid "pattern differentiation and formula matching" mindset of conventional TCM. By integrating modern medical research on the regulatory mechanisms of the human immune system, I have independently constructed a complete theoretical system: "Difficult and Complicated Diseases and Immune System Dysregulation Syndrome." This framework abandons the previous approach of treating various complex疑难杂症 in isolation. Instead, it centers on the core issue of overall immune imbalance in the human body, re-establishing a unified pathogenesis for diverse, cross-system perplexing symptoms. It builds a complete diagnostic and therapeutic framework encompassing pathogenesis analysis, pattern-based treatment, and long-term maintenance. In numerous difficult cases where conventional therapies have shown little effect, this theory has achieved stable and lasting clinical outcomes, opening up a fundamentally new core pathway for TCM to overcome疑难杂症.
I. The Common Essence of Difficult and Complicated Diseases: Overall Imbalance of Immune System Dysregulation
Over decades of clinical practice, I have observed a commonality among a large number of疑难杂症 cases that modern medicine classifies as "of unknown etiology": the patients' symptoms are scattered across multiple bodily systems, neither conforming to the characteristic lesions of a single organ nor fully explainable by any one disease diagnosis. Patients often visit multiple specialists, yet their symptoms recur without radical cure. These patients frequently present with concurrent cross-system manifestations such as long-term recurrent skin allergies, unexplained joint aches, persistent low-grade fever and fatigue, severe sleep disorders, and digestive disturbances. Various examinations reveal no clear organic lesions, and they are ultimately vaguely labeled as "sub-health" or "autonomic nervous system dysfunction," lacking targeted and effective treatment.
Based on this, I clearly propose: the fundamental problem in such疑难杂症 is not local organ damage, but a holistic functional disorder of the human immune system. This disorder is not equivalent to the common perception of "lowered immunity," but rather a comprehensive imbalance of immune mechanisms: some immune pathways are overactive, mistakenly attacking the body's own normal tissues and inducing various sterile inflammations and allergic reactions; other immune pathways are persistently suppressed, failing to clear metabolic waste and residual pathogenic factors in a timely manner, allowing minute lesions to linger and fail to heal. This systemic immune imbalance spreads through the channels of qi and blood to all the viscera and systems throughout the body, ultimately manifesting as various perplexing symptoms in different parts of the body. This is the shared root pathogenesis of all cross-system疑难杂症.
Interpreted from the TCM perspective, overall immune dysregulation corresponds to the complete disruption of the body's healthy qi (zheng qi) equilibrium. Traditionally, healthy qi does not simply mean fullness of qi and blood; more importantly, it emphasizes the dynamic coordination within healthy qi itself. Defensive qi (wei qi) governs surface defense, while nutritive qi (ying qi) governs internal nourishment. The two mutually constrain and coordinate to maintain normal immune function. Long-term overwork, emotional stress, improper medication, and environmental toxins continuously deplete the body, causing the originally balanced state of ying and wei to collapse completely. Excessive wei qi leads to allergies, redness, swelling, low-grade fever, and other excessive immune reactions; deficient wei qi results in recurrent colds and pervasive fatigue; disordered ying qi causes qi and blood stagnation, leading to joint pain, tissue proliferation, and other issues. This overall ying-wei imbalance cannot be summarized by any single pattern such as qi deficiency, blood deficiency, yin deficiency, or yang deficiency. This is precisely the key reason why conventional pattern differentiation often fails to cure such difficult diseases radically.
I emphasize here that many previous practitioners, when treating such疑难杂症, would only address individual symptoms locally—merely suppressing allergies for allergies, simply relieving pain for joint pain, or piling up qi- and blood-tonifying formulas to blindly boost immunity—without ever touching the core pathogenesis of overall immune imbalance. The result is only temporary relief of discomfort, without eradicating the root cause. In some cases, excessive intervention further disrupts the immune balance, making the condition increasingly stubborn and complex.
II. The Three-Level Pathogenesis Evolution Pattern of Immune System Dysregulation Syndrome
Through tracking the complete disease courses of thousands of patients with疑难杂症, I have divided the pathogenesis evolution of immune system dysregulation syndrome into three progressive stages. Each stage has its own characteristic symptoms and core pathogenesis, fully aligning with the natural, gradually worsening process of疑难杂症 and providing a clear, traceable logic for the otherwise chaotic and complex symptoms.
First Level: The Initial Chaos Stage of Ying-Wei Disharmony
This is the earliest phase of immune dysregulation. Most patients have clear precipitating factors: excessive use of antibiotics or corticosteroids after a severe cold, continuous months of staying up late with overwork, or experiencing a major emotional blow, all of which directly disrupt the coordinated operation of ying and wei qi at the body surface.
Typical manifestations at this stage include recurrent unexplained urticaria, skin itching, susceptibility to colds when exposed to wind, persistent low-grade fever, fear of wind and cold, migratory body pain, and unremarkable findings on various instrumental examinations. At this point, the immune dysregulation remains only at the body surface and has not yet invaded the internal organs, making it the optimal period for intervention. However, most patients are misdiagnosed with common allergies or colds and receive long-term suppressive medications, which trap the pathogenic factors that could have been dispersed outward from the body surface, driving the condition toward deeper levels.
Second Level: The Mid-Chaos Stage of Qi and Blood Stagnation in the Triple Burner (San Jiao)
If the pathogenic factors from the initial stage are not properly dispersed, they invade inward along the triple burner channels, causing extensive stagnation throughout the body's qi and blood circulation pathways. The immune dysregulation spreads from the body surface to all major systems.
At this stage, patients present with a complex array of symptoms, with abnormalities in multiple systems appearing simultaneously: persistent chest tightness and abdominal distension, poor digestion after eating, alternating loose stools and constipation; accompanied by severe insomnia, anxiety, and unexplained palpitations; aseptic pain in multiple joints; recurrent stubborn eczema and subcutaneous nodules on the skin; some individuals may show thyroid dysfunction or mildly elevated autoantibodies. At this point, the immune system is thoroughly imbalanced, with overactivated immune cells continuously attacking the body's own mucosal and joint tissues, while the immune pathways that should perform defensive functions remain suppressed. Conventional formulas for soothing the liver, strengthening the spleen, or tonifying the kidney are insufficient to clear the extensively obstructed qi and blood pathways. The condition recurs year after year, lingering for years or even over a decade without resolution.
Third Level: The Deep Chaos Stage of Yin-Yang and Qi-Blood Reversal
If the mid-chaos stage conditions are left untreated for too long, they ultimately lead to a complete reversal of yin, yang, qi, and blood, with the immune system totally losing its self-regulatory capacity. Some patients progress to definite autoimmune diseases such as refractory rheumatoid arthritis, systemic lupus erythematosus, Sjögren's syndrome, etc.; others suffer from years of profound fatigue, unable even to complete daily activities such as getting up and moving around, with multiple unexplained nodules and tissue proliferations appearing throughout the body, and some even gradually progress to severe pathological changes.
The pathogenesis at this stage is extremely complex. Patients simultaneously present with severe deficiency and deep stagnation: they are both afraid of cold and prone to frequent oral ulcers and heat signs. Using warming yang herbs alone immediately causes上火 (excessive heat symptoms), while using heat-clearing herbs alone causes diarrhea and loose stools. Conventional pattern differentiation often falls into a dilemma, making this the most clinically challenging category of疑难杂症.
III. Core Distinctive Therapeutic Approaches for Regulating Immune System Dysregulation Syndrome
Based on the characteristics of the three progressive stages of immune system dysregulation syndrome, I have developed a distinctive therapeutic system that differs from conventional approaches. The core philosophy of this system is neither to simply "boost immunity" nor to blindly "suppress immunity," but rather to harmonize ying and wei, dredge the triple burner, and balance yin and yang as the fundamental principles, progressively restoring the immune system's own dynamic equilibrium and resolving the core contradiction underlying various疑难杂症.
For the first level, the initial chaos stage of ying-wei disharmony, I establish the core therapeutic method as unblocking and harmonizing ying and wei to expel pathogenic factors outward. Throughout the entire process, pure tonics and bitter-cold heat-clearing herbs are strictly prohibited. Clinically, I often use light combinations of Cinnamomi Ramulus (Gui Zhi) and Ephedrae Herba (Ma Huang), together with Saposhnikoviae Radix (Fang Feng) and Schizonepetae Herba (Jing Jie), to unblock the stagnant ying and wei qi at the body surface, allowing the early-invading pathogenic factors to be naturally dispersed outward through the skin and muscles, and rapidly restoring the harmony of ying and wei. For most patients in the early stage of immune dysregulation, after several weeks of medication, symptoms such as recurrent allergies, low-grade fever, and fear of wind completely subside, immune function quickly returns to balance, and the disease progression is directly halted.
For the second level, the mid-chaos stage of qi and blood stagnation in the triple burner, the core therapeutic method is defined as differentially draining the triple burner and regulating qi and blood. Instead of focusing on a single viscus, this approach simultaneously unblocks the stagnant qi and blood pathways throughout the upper, middle, and lower burners of the entire body. I commonly use Platycodi Radix (Jie Geng) and Aurantii Fructus (Zhi Qiao) to open and disperse the qi mechanism in the upper burner, Pinelliae Rhizoma (Ban Xia) and Magnoliae Officinalis Cortex (Hou Po) to resolve stagnation in the middle burner, Poria (Fu Ling) and Alismatis Rhizoma (Ze Xie) to drain dampness from the lower burner, and add small amounts of Salviae Miltiorrhizae Radix (Dan Shen) and Chuanxiong Rhizoma(Chuan Xiong) to gently unblock the blood vessels throughout the body, gradually opening the blocked qi and blood pathways. Treatment at this stage does not seek rapid results; rather, it focuses on progressively unblocking the pathways, allowing the disordered immune cells to return to their normal tracks of qi and blood circulation and cease their unwarranted self-attacks. Many multi-system perplexing symptoms that have persisted for over a decade gradually resolve one by one with sustained treatment, and the immune status progressively stabilizes.
For the third level, the deep chaos stage of yin-yang and qi-blood reversal, the core therapeutic method is simultaneously regulating cold and heat to balance yin and yang, stepping outside the binary "either warm or cold" thinking. Based on each patient's actual pathogenesis, I precisely proportion warming yang herbs like Aconiti Lateralis Radix Praeparata (Fu Zi) and heat-clearing herbs like Coptidis Rhizoma(Huang Lian) to simultaneously harmonize the internally disordered cold and heat, supplemented with various herbs that mediate and harmonize yin and yang, gradually re-establishing the orderly balance of yin and yang in the body. The treatment course at this stage is relatively long, but once the reversed yin and yang are properly regulated, the completely失控 (out-of-control) immune function can slowly regain its self-regulatory capacity. Many patients with severe autoimmune diseases, after long-term treatment, can gradually reduce or eliminate their dependence on hormones and immunosuppressants, achieving long-term stability without relapse.
The theoretical system I have proposed—Difficult and Complicated Diseases and Immune System Dysregulation Syndrome—breaks through the previous limitations of treating various cross-system difficult diseases in isolation. By establishing a unified pathogenesis understanding and complete therapeutic framework centered on the overall balance of the human immune system, it provides a clear and actionable roadmap for curing many previously untreatable疑难杂症. It also opens up a new academic direction for TCM diagnosis and treatment of difficult diseases.
